Create Carer Account

To use mindminder you will need to create a "Carer" account by filling in the form below.
All data is handled in accordance with the mindminder Privacy Policy and Data Retention Policy.
Username Your username will be the identity you log-on with.
Usernames can be an EMail address. Usernames must be unique between Carers.

A 'familiar' name is one you may be known as within your Organisation.
Familiar names are only used for Carer forms within dashboards and for correspondence.
Carer Type
Carer types help mindminder understand the nature of carers to help tailor the design development.
If you may be in more than once category, select the one that you'll be spending most time (majority of Users).
