Create new Carer profile
To use mindminder you will need to create a "Carer" account by filling in the form below.

A mindminder Carer is the way an Organisation can interact with its Users.
Carers must be a member of an Organisation to interact with Users within that Organisation.
As a Carer you may be:

  • A Family member: Managing mindminder interactions for (for example) a parent User
  • Care Home staff: Managing mindminder interactions for User residents
  • A GP or health professional: monitoring patient User data
  • An individual acting as both Carer and User for self-monitoring

Note also that if acting as Carer AND Organisation, separate Usernames will need to be provided as mindminder treats Organisations and Carers as separate entities.

All data is handled in accordance with the mindminder Privacy Policy and Data Retention Policy.

Account Details
  Your username will be the identity you log-on with. Usernames can be an EMail address. Usernames must be unique between Carers.

  Your 'Familiar Name' is one you'll be referred to by within mindminder; for example Dr Susan Jones may be known within an organisation as 'Dr Sue'.
  A default Avatar will be used if none supplied (can be updated later)
  This is a phrase used to securely identify a Carer when interacting with a User account. A default 3-word passphrase (one of 485,587,656 combinations) is provided above however you can select an alternative if preferred. Change Passphrase

Carer Demographic
  Knowing Carer demographics helps understand and inform mindminder's development.
  Carer types help mindminder understand the nature of carers to help tailor the design development. If you may be in more than once category, select the one that you'll be spending most time (majority of Users).
 
 

To interact with Users via mindminder you must also be a member of an Organisation.
You can join an existing Organisation now (if you have the relevant details), create a new Organisation, or proceed only with a new Carer account for now (Organisations can be created/joined later).
